New Clients

Welcome to CCCA. I hope that the material contained on this website will provide important information and answer some questions prior to your first visit. If you wish, you will be able to complete most of the necessary paperwork in advance of our first meeting then I suggest you do the following:

1. Download and complete the registration form.

2. Please read the Confidentiality Information form or the HIPPA form and I will briefly discuss them with you when we meet. After I answer any questions you might have I will ask you to sign an  informed consent form and the paperwork will be finished. 

If you prefer, we can complete the paperwork at your first session. I will be happy to answer any questions you might have regarding this information when we meet. Below is a downloadable list of all of the documents.

  This is two pages of information detailing aspects of therapy and practice policies.

• Confidentiality Information
  There are many complicated confidentiality issues that you should know about, so we prepared three pages of information for you.

• Registration Form
  This is a client registration form.

• HIPAA Notice Form
  This notice describes how your Personal Health Information (PHI) can be used and disclosed and how you can get access to that information. Please review it carefully.

• Informed Consent Signature Form
  This form indicates that you received the HIPAA notice form and are aware of how your PHI will be used for the purpose of treatment, billing and health care operations. Some, but not all, of this information is covered in the confidentiality information form. Please review it carefully.
